Bilingual Community Based Case Manager
Full Time
Hours: 7:30am-4:00pm, Monday-Friday with occasional evening and weekend hours for outreach events.
Keystone Health is seeking a full-time Bilingual Community Based Case Manager to join the team.
The Bilingual Community Based Case Manager will be primarily responsible for collaborating with the Chambersburg Area School District in the development of a Welcome Center in downtown Chambersburg. There will be an allocated office in this center for this person to meet with families and assist with the education about and navigation of the health care system and community resources.
Responsibilities include promoting access to medical care, ensuring incoming students are up to date with vaccines and well child checks based on school requirements, connection to insurance or reduced fee programs, evaluation of SDOH and facilitating referrals to appropriate resources. This position will also work with Keystone Health patients and community members who need assistance with SDOH needs and attending outreach events as appropriate.
Qualifications: Bachelor’s degree in social work or human service-related field and at least one year experience and/or training with human or community services, or any equivalent combination of education and experience. Community outreach experience preferred.
Language Skills Requirement: Must be able to read, write, and speak Spanish or Haitian Creole and English and proficiently.
